You can designate someone to pick up food for your family. Please write a note saying:
“I authorize (fill in name) to pick up food from the Athens County Food Pantry for me (fill in your name and birthdate).”
Sign the note.
Have your proxy bring a picture of your ID (as an image on a phone, for example) and their ID. Have your proxy bring evidence of your address in Southeast Ohio (a picture of a utility bill on a phone, for example). Ask for a proxy form, fill it out and return it, so we can have it on file.
